Case Manager – Veterans Brain Health & Suicide Prevention Program

Salary range: $50,000 – $58,000 (Grant Funded), commensurate with experience.
Position Overview
The Case Manager will provide direct support to veterans participating in The Mac Parkman Foundation’s brain health and suicide prevention program. This role is responsible for conducting clinical intake and needs assessments, coordinating referrals to treatment and support services, maintaining regular contact with participants, and ensuring program compliance with all grant requirements. The Case Manager will collaborate closely with clinical partners, outreach staff, and the leadership team to ensure veterans receive timely, appropriate, and holistic support that addresses both immediate needs and long-term recovery goals.
Key Responsibilities
Client Services
- Conduct comprehensive intake assessments to identify participant needs, including brain health concerns, mental health challenges, housing, employment, and other support services.
- Develop individualized service plans in collaboration with each participant, setting measurable goals and identifying appropriate resources.
- Provide ongoing case management through regular check-ins, both in person and virtually, to monitor progress and adjust plans as needed.
- Facilitate referrals to treatment providers for innovative brain health therapies, mental health counseling, substance use services, and other supports.
- Assist participants with travel questions on transportation for treatments or service appointments when necessary.
Program Coordination & Compliance
- Maintain accurate, timely, and confidential case files in compliance with HIPAA and organizational data policies.
- Track and document service delivery, participant progress, and outcomes for grant reporting.
- Participate in multidisciplinary team meetings to review cases and coordinate care.
- Ensure all services align with the project’s suicide prevention and brain health objectives.
Partnership & Outreach
- Collaborate with community organizations, VA facilities, veteran service organizations, and healthcare providers to strengthen the referral network.
Minimum Qualifications
Required:
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field; or equivalent relevant experience. MSW or LCSW a plus.
- Minimum of 2 years’ experience in case management, veteran services, or related human services role.
- Knowledge of veteran culture, military service impacts, and community-based resources in Florida.
- Strong organizational skills, with the ability to manage multiple cases and priorities.
- Excellent interpersonal and communication skills.
- Proficiency with Microsoft Office Suite, database systems, and virtual meeting platforms.
Preferred Qualifications:
- Experience working with traumatic brain injury (TBI), PTSD, or mental health programs.
- Familiarity with suicide prevention best practices and crisis intervention strategies.
- Military service or military family background strongly preferred
